Nutricia Homeward Transition Support Service

When young adults move from children’s NHS services to adult NHS service and start to take more responsibility for their enteral nutrition, they may have many questions. We have developed literature to answer some of those questions, and to help make this transition easier for both the patients and their parents and carers.
